Step 1 - Get the form
Create an account on the Australian Passports Office website and follow the prompts to generate a form. Once you have answered all the questions, you will be able to download and print the form.

Step 2 - Print, sign and date your form
Print: You may need to adjust your print settings so that the top/bottom of the pages are not cut off, especially the barcodes and the Office Use Only section at the bottom.
Sign: Make sure you sign with a black pen inside the white box. This signature will be cropped to use in your new passport, so if part of your signature is outside the box, your application will be rejected.
Date: Use Australian date format (DD/MM/YYYY). Signatures are valid for six months only. An application signed more than six months ago cannot be accepted.
Please do not use white-out or correction fluid or trace over your signature with a black pen if you signed in blue first. If you have made a mistake, please edit the form on the Australian Passports Office website and reprint the form.
Step 3 - Photographs
You will need three Australian standard passport photos. Please refer to the photo guidelines.
Do not take your photos at a photo booth, go to a photographer. Please discuss the requirements with the photographer and request the unedited digital format of your picture along with the hard copies.
Photos must be less than six months old.
Write your name on the back of one photo in black ink. Ensure no ink is smudged and that the ink does not transfer to the other photos.
Do not attach the photos to to the form with paperclips, staples, tape or glue. Leave them loose and we will affix them to your form.
Step 4 - Payment
Print out and complete our payment authorisation form (PDF). We accept debit or credit cards, Visa or MasterCard. When we receive your application, we will charge your card.
Please consult the passport fees and remember to include the appropriate courier fee to send you your new passport if you don't want to collect it in Madrid. Please note that we can only send passports within Spain and Andorra.
Step 5 - Lodging your application
Send us your application with all the below:
- Application form
- Three photos
- Payment form
Do NOT send us your current/expired passport or any other original documents. You may still use your current passport until you receive the new one.

It is important that you:
- do NOT fold your application
- do NOT use staples, paper clips or anything else that may damage your photos
Processing
Please allow 6 weeks for your application to be processed. Some applications may be processed faster but we cannot guarantee anything less than 6 weeks.
Once we receive your new passport, we will send it via DHL courier to your address, if you selected the courier option. If you indicated that you want to collect your passport at the embassy, we will email you to arrange an appointment for collection.
Please note, your old Australian passport will be automatically cancelled 30 days after your new one is issued. Using a cancelled travel document may be a criminal offence under subsection 32(1) of the Australian Passports Act 2005. This attracts penalties of imprisonment or a fine or both. If you will be out of the country for longer than 30 days after your new passport is due to arrive, please let us know in advance.
Urgent travel
There is no expedited service for applications lodged overseas, this service is only available within Australia.
If you need your passport in less than six weeks, you may be eligible for an Emergency Passport. Evidence of urgency (i.e. travel itinerary, visa appointment confirmation or letter from your employer) is required.
